Requests

As standard procedure, please provide the following information to assist in scheduling laboratory work. Payment arrangements must be made before sampling materials will be shipped. Your request should include:

  1. Number of samples
  2. Type of analyses
  3. Date of first sampling
  4. Contact information
  5. An Authorization for In-Service Agreement (ISA) including a job code for billing
