The official forms repository is now at Grants.gov at http://www.grants.gov/agencies/approved_standard_forms.jsp#1.  Some of the most commonly used forms are listed below. 

If you select a PDF formatted file, you must have a recent version of the Adobe Acrobat Reader or Adobe Acrobat properly installed on your computer to be able to use these forms. You may obtain Acrobat Reader directly from Adobe's Web Site at no charge.

Using Forms:
 

Adobe files are fillable and printable with the free Adobe Reader, but you will not be able to save your completed form unless you have Adobe Acrobat.
MS Word and Excel files are version 2000 unless otherwise noted.

Annual Reports:  In accordance with 7 CFR 3016.40 and 7 CFR 3019.51, Performance and Financial Status Reports are required, annually at a minimum. 

Recipients are allowed 90 days from the end of the reporting period to submit their reports.  The reporting period is based on the Federal fiscal year which ends September 30.  All reports must be received by the Forest Service contact listed in the agreement, no later than December 31.
