The Flathead National Forest will be the central point for obtaining and maintaining a listing of resources and preparing subsequent Emergency Equipment Rental Agreements.
All personnel dispatched to fire incidents must have a current Standards for Survival certificate.
AD (Casual) Hires: All AD hires are required to submit proof of citizenship at the time of hire. You will need to have a Driver's License and either your Social Security Card or a Birth Certificate. A U.S. Passport can be substituted for the Social Security Card and Birth Certificate, if available. You'll also be required to do Direct Deposit so will need Direct Deposit information; i.e., account number, routing number, and bank name and address.
Pickup Drivers: A current copy of the vehicle registration is required for all pickups, as well as the completed Resource Sheet and the current Standards for Survival Certificate. If you are currently covered under Worker's Compensation coverage or have an Independent Contractor Exemption Certificate, all you'll need is a copy of the Certificate. If you do not currently have Worker's Compensation coverage/exemption, you will be picked up as an AD hire and you will be required to submit documents as discussed under "AD (Casual) Hires."
Sawyers/Swampers: In addition to the completed Resource Sheet and the current Standards for Survival Certificate, please bring the vehicle registration for your sawyer vehicle. You'll be required to have completed a Light Physical Fitness Test prior to being hired as an AD Sawyer/Swamper. To be hired as an AD, you'll be required to submit documents as discussed under "AD (Casual) Hires."
Heavy Equipment: In addition to the completed Resource Sheet and the current Standards for Survival Certificates for all operators, please have your insurance company submit a current Worker's Compensation Certificate to us. They can mail it to: Flathead National Forest, 1935 Third Avenue East, Kalispell, MT 59901 or fax to us at: 758-5326. If you operate under an Independent Contractor Exemption Certificate, please submit a copy with the Resource Sheet.
If you previously had an Emergency Equipment Rental Agreement with the Flathead National Forest, please bring it with you noting any changes.
