6730 - ACCIDENT REPORTING AND INVESTIGATION

6731 - ACCIDENT INVESTIGATIONS

            1. Timely and accurate reporting is mandatory.

            2. A summary of accidents shall be reviewed by the Forest Supervisor and Staff at Staff Meetings on a monthly basis.

            3. All vehicle (private party or over $1,000 Government property damage), fatalities, serious injury, and major property damage accidents shall be investigated by a Chief Investigator and the appropriate District Ranger, Center Director, or Forest Staff Officer. The Forest Criminal Investigator shall participate in the investigation on an as needed basis. The role of Chief Investigator shall rotate among Forest Staff Officers as shown in Exhibit 1. The Chief Investigator shall be notified by telephone when an accident occurs and accidents shall be investigated no later than the next workday.

            4. The scheduled Acting Forest Supervisor shall substitute if the Chief Investigator is not available. If neither of these are available, contact the Forest Safety Coordinator.

            5. In the event of the Chief Investigator and the appropriate Forest Staff Officer, being one and the same, a member of the Staff Officer's staff shall represent him in the capacity of Forest Staff Officer.

6732 - ACCIDENT NOTIFICATION, REPORTING, AND REVIEW. The Chief Investigator (See 6733) shall be notified immediately by telephone of any lost time, automobile (private party or over $350 Government property damage), fatalities, serious injury, and major property damage accidents. If any of the conditions identified in FSM 6732.11 or R8 supplement to the parent text shall exist, one of the following persons in the order shown shall be notified by the Chief Investigator.
